THE JOB
As a Care Manager, you'll help individuals who are actively working to address their mental health and substance use challenges. You will assist individuals in our services to find safe and stable housing, access local resources, medical care and medications, employment or financial resources, social support, and more. As an active member of the treatment team, you will work alongside medication prescribers, licensed therapists, crisis response teams, and various agencies and organizations throughout the community to help the participants obtain the services and resources they need. As each individual continues along their recovery journey, you'll provide support, crisis intervention strategies, and will work directly with each individual to adapt their treatment plan to fit their unique strengths, needs, and goals as they arise.

The position is full-time, Monday through Friday. Benefits are included with full-time employment.

WHAT YOU HAVE
Required:
- High school diploma or equivalent

- Two or more years of experience working in Behavioral Health

- Knowledge of co-occurring disorders and related needs

- At least 21 years of age

- Valid driver's license with acceptable DMV record (last 7 years)

Bonus Points:
- Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred
